EMERGENCY CARD ~ VERY IMPORTANT
An emergency card for every child at Hart Street School is required in the school office. It should include three telephone numbers to insure contact with a responsible person when your child become ill or hurt. The cards are sent home at the beginning of the school year for your completion. Please read both sides of the card carefully and complete all information. Be sure to sign the card, and return it to school with your child immediately. If your emergency information changes during the school year, please notify the school. Please remember we WILL NOT release your child to anyone whose name is not written on the emergency card.

ABSENCE FROM SCHOOL:
We hope that all of our students strive for a perfect attendance record this school year. However, should your child miss school due to illness, a note from a parent must accompany every child on his/her return to school. A special absence card is available and will be sent home with every child if he/she does not bring a written note. State Law requires parent verification of their child's absence. If the absence is do to illness, the nature of the illness should be stated. Each time a child returns to school he/she must present the written verification of absence from the parent to the teacher. A child who has been ill for three days or more should report to the school nurse for readmittance upon arrival at school.

A Pupil Service and Attendance Counselor is at Hart Street one day per week to monitor student attendance. Students with excessive absences will be contacted by that office within the district, so that they may assist parents and insure that students are here and on time daily.
